Child Mental Health ServicesThe Child Mental Health Program provides a comprehensive array of community-based treatment and support services to children with Serious Emotional Disturbances and their families that allow these children to remain in the safest environment, where they can reach their optimal potential. Clinic Plus Treatment for Children and Adolescents - Children enter Clinic Plus through the screening process or through the regular referral process. All children entering Clinic Plus receive a Comprehensive Mental Health Assessment. This assessment can take up to 3 visits to complete. After the assessment is complete a service plan is developed with the family, addressing their strengths and needs. A crisis plan is developed if needed and the transition plan begins as soon as services start. Up to 9 in-home visits can be offered to the family to ensure that the mental health treatments offered work within the real-life environment of the chills and family. On-going training in evidence based practices is occurring to ensure the child and family receives treatments which are empirically proven to work. Cultural Competency is imbedded within all phases of Clinic Plus. Children in Clinic Plus have access to psychiatric consultation and medication if indicated. All children enrolled in this program have a serious emotional disturbance. All children and families participate in a strength and cultural discovery process in order to form a child and family team, comprised of paid professionals, community providers and natural supports. The family identifies the members of their team. The child and family team determines the supports and services required to meet the needs of the family, as well as help the family move from paid professional services to community and natural supports. The Intensive Care Coordinator acts as the facilitator of the process. 1131 Broadway Buffalo, New York 14212 Phone: (716) 852-0383 Home and Community Based Services Waiver Program - Provides service coordination for children who are at the highest risk of out-of-home placement. In partnership with other child agencies, HCBS offers intensive support and monitoring through home visits, respite, skill building, crisis intervention and family support services. Individualized Care Coordinators are available 24 hours/day, 7 days/week. All services take place in the families' home or community. 11 West Main Street Lancaster, New York 14086 Phone: (716) 681-5077 School Based ProgramSchool Based Mental Health Program - is a collaboration between New York State Office of Mental Health, New York State Education Department, Erie County Department of Mental Health and the United Way-Closing the Gap. The four components of school based mental health services are: · The integration of mental health supports and services into the regular education processes for identifying at-risk children, as well as into the processes for planning and delivering services to the popluation. · The development or enhancement of family support mechanisms within the school. · Training for school staff, families and mental health professionals in the areas of individualized care planning and strengths based approaches; behavior management; family/professional collaboration; recognizing the symptoms of childhood emotional and behavioral disorders, and effective strategies for addressing these disorders. · The provision of school based clinical services. The team is comprised of a kinship services facilitator and an attorney from Neighborhood Legal Services. Families coming to the Court in crisis will be able to have immediate access to support and services. The team will walk a family through both the process and the system until their custodial decisions and arrangements have been made. They will also follow up with linkages to existing community resources and monitor that families have been able to access them. This program is housed at Family Court.
